Purpose of Form I-693 (Report of Medical Examination and Vaccination Record)
Form I-693 is the medical examination required by U.S. Citizenship and Immigration Services (USCIS) to make sure an applicant does not have any health-related conditions that would make them inadmissible to the United States.
A USCIS-approved civil surgeon must complete the exam and fill out the form.
🩺 Why Form I-693 Is Needed
USCIS uses the I-693 to confirm that the applicant:
1. Meets all medical requirements for a green card
This includes screening for:
Tuberculosis
Syphilis
Gonorrhea
Physical or mental disorders that may pose risks
2. Has received the required vaccinations
These may include:
Measles, mumps, rubella (MMR)
Varicella (chickenpox)
Tdap
Hepatitis B
COVID-19 (as required by CDC guidelines)
If vaccinations are missing, the civil surgeon will update them during the visit.
📌 When Form I-693 Is Required
You need Form I-693 in any green card process, including:
Form I-485 (Adjustment of Status)
Some humanitarian cases
Certain employment-based adjustments
If the applicant is doing consular processing abroad, the medical exam is still required—but the embassy uses a different form, not the I-693.
📦 How and When to Submit It
Applicants have two options:
Option 1 – Submit with the I-485
This can speed up the approval process.
Option 2 – Submit later when USCIS requests it
USCIS may send a Request for Evidence (RFE) asking for the I-693 before they can approve the green card.
🛑 Important
The I-693 must:
Be completed by a USCIS-designated civil surgeon
Be signed properly
Be submitted in a sealed envelope (USCIS will reject an opened one)
